Overall the facility and staff are great. The facility is clean and bright. The staff is friendly and caring. There have been some hiccups, but they were addressed quickly. They take safety and health seriously, and the activities are nice. The only negatives are really (1) cost, it's more expensive than other comparable facilities and I can't exactly pin point why, and (2) the food options have not been great (my Mom often complains about it, so we have to supplement). Lastly, although I can't say it's anyone's fault, but there is almost no diversity in residents. My mom is one of 3 black residents, and the only one on the assisted living side of the facility.

Based on the experience so far, we do like some of the staff and my mom enjoys them and some of the other residents there. However we do believe that some of the staff turnover is causing some issues. communication regarding health status of mom was lacking.

The staff was nice but under staffed in all areas, activities far and few between, very little stimulation mental and physical, medical nurses were uniformed as to what meds were for and side effects. It wasn't an experience I would want a family member or myself to go through.

At the time my mother moved in I would have ranked them a 5, but now it is a 2. Since the beginning of the year Prema has received very poor scores from the county health department due to multiple violations of food storage and food preparation. The county's required report, although it is mandatory the facility post the reports, they are routinely posted in an area where only employees access. These reports are not visible to anyone visiting the facility for a tour. There have been issues with cleanliness of the facility and lack of staff to assist residence. They have discontinued a number of activities due to lack of staff. They routinely use employees from an employment agency to prepare food, administer the resident's medications and their website provides misleading information regarding the quality of their food. Management staff rarely communicates with the residence or their family members, and when concerns are brought to their attention, they provide misleading information regarding how they intend to address the issues.

I'd say it requires vigilance on the part of the family to get adequate care. There has been a fair amount of turnover at leadership and staff levels. Experienced delayed notification of a fall that required hospitalization. Some staff are very friendly and attentive, others seem to do the bare minimum. I'm beginning to believe this is an industry-wide issue (this is our third location and had similar concerns at the others).

I have a loved one at this location. I choose this facility as it was close to home and the cost. Initially the experience was good with the previous Executive Director (ED). Since the new ED has joined the place has gone hell in a hand basket! There is a lot of favoritism as she brought in her friends and protects them. Even after several complaints from many family members care staff who is in her inner circle does not get fired. If you complain you get nasty emails from her telling you why you are wrong instead of solving the problem and circling back with the family. Her front office staff is extremely disrespectful and will argue with family members to prove they are right. They do not de-escalate the situation but add more fuel to the fire. The staff forgets we have our loved ones in their care. We have a right to complain when their staff does not follow the care plan they promised us when we brought our loved one to the facility.

I had my Godfather at Prema at Ashton Hills for a month and a half in Memory care. He was not easy to deal with at all. But I was contacted by the nurse and told we needed a meeting. When we arrived, we were told by that nurse that we would have to take him with us that night imperative after the so-called meeting. We were not prepared at all and were blindsided. How do you get kicked out of memory care? That is what it is for along with the extra cost. We were screwed and had a heck of a time. While we were happy with the Head person in memory care and the person who placed us. We were very unhappy with the nurse. She was unprofessional to say the least. I would never do business with them again. They were concerned about another patient with a possible lawsuit and didn’t want us to talk to them or have them overhear anything. Instead of fixing the problem as you would think memory is for, they kicked my godfather out without prior notice. Not happy with the upkeep of his room.

The review is for the assisted living facility only. I have heard good things about the memory care side which I think is the only reason this facility is still limping along and barely operating on the assisted living side. In two years: 4 Executive Directors, 3 Sales Managers, 3 Activity Directors, 3 cooks. Exceptional care staff and med techs are thankfully able to provide care in spite of management incompetence. Communication with residents and families is limited to email. Concerns are never addressed, resolved, or improved. Food is terrible. Morale is at an all time low among residents. Many have left with more leaving. Management only interested in offering excuses if you can management to speak to you at all. Lots of liability issues being covered up and ignored. Every facility in the area is short staffed and has the same issues but other facilities seem to have management in place to communicate and address the issues and work to resolve problems.

My Dad was a resident at The Oaks at Suwanee Creek for a few weeks. I cannot recommend this facility. My dad had a serious fall which the nurse in charge of memory care and staff failed to properly care for or report to our family. The result was that Dad sat at The Oaks at Suwanee Creek with a broken hip and in tremendous pain for over 48 hours. During those 48 plus hours, my brother and I tried to get answers about Dad's change in condition. No one at The Oaks told us of the fall or gave us any other information of use. It was not until I made the decision to get my dad to the hospital that I finally got answers. We were quickly told that dad had a serious hip fracture. Some time later, the director of The Oaks called me to say, "your dad did have a fall and the nurse says she "forgot" to call you". My dad suffered needlessly for over 2 days because someone "forgot". This failure in communication and lack of attention to my dad's obvious signs of pain were inexcusable.
