HomePlace Memory Care at Oak Harbor

Home Place Oak Harbor is a unique home-oriented Alzheimer's and Dementia Care Facility. We specialize in providing the highest quality of care to our resident's, by employing caregivers and nurses who are kind, compassionate and educated to this level of care.
We have 3 living areas with a total capacity for 50 residents.

Meadows has a capacity for 18 residents, and it is for those needing the highest level of care, and occasionally, it is for residents who need a quiet and less stimulating environment. Most of these residents require total assistance with their activities of daily living.

Woodlands has a capacity for 16 residents, who are mostly independent with walking, and with much of their personal care. They need a secure environment to ensure they do not wander away.

Seaside has a capacity for 16 residents. These folks are in-between independent and heavy care. They are usually very busy, and may need our special dementia redirection, or distraction, skills.

The evening he went to the hospital he was severely dehydrated and I also took pictures of him with bruises in his ears , unfortunately he never left the hospital the food was very good and the girls that took care of the patients were very nice and caring. I think they need more caregivers on the floor a lot of times there's only one girl on the floor.

The right place
I stumbled on to this facility. It was a sister facility of the assisted living facility my mom was in, but I had no idea they had other facilities and one dedicated to memory care. When the facility she was in said they had no room in their memory care unit, I called around to other facilities, when a person at a facility in Burlington who had no room clued me into the 5 facility network connected to where my mom was currently living, I called home place and set up a meet. Oak harbor was 45 mins away, but the staff, appearance and price difference made it my choice for my mother. It is still difficult to place your loved one anywhere, but when memory issues are involved, no where is ideal, but Home place was the best for the money. It has been a great choice. My mother is adapting and my frequent visits show her to be clean and cared for. I am grateful I found Home Place and after dealing with two other facilities and staff, home place has the best staff I have worked with to date.

Things to know about HomePlace
The overall experience would have been very good. It was not rated higher because they required 2 year full payment before accepting Medicaid. The floor plan was excellent for social gatherings, very open, and did not feel confining like a house or hospital. One concern is that there are patients with fairly severe mental issues who cry or yell constantly. The staff is always there for them, calming and tending, but that usually did not help. Not sure what they could do differently, but it tended to bother other residents and make what was a very pleasant area fairly stressful.

My first experience with the staff was when I went for a tour. They spent a lot of time with me showing me around and answering all of my questions. That was not like other facilities I toured. I felt like I had a lot of information before I even decided. Regarding communication with the staff, [names removed], who answer the phones were always really helpful in hooking me up with whoever I needed to talk to, and most of the time those people were available. So I always felt that I could have my issue or question answered very quickly. I was really impressed with the staff members who actually took care of the clients. The neighborhood concept was fantastic and I think that the staff really got to know the clients well. I watched one young man catch a movement of one of the women who was moving out of her chair, which was dangerous for her, and he immediately went to her, helped her and was so kind and loving in a very appropriate way to support her. It was actually quite moving. I always felt that my husband was safe, even though I think he was a difficult patient with his type of dementia. I have no reservations about recommending this facility.

The staff was good other than that I would give them a #3 except for the quality of care he was constantly falling and even had bruises on the upper part of both ears I have the pictures the in house doctors saw him on a Tuesday morning and that evening he landed up in the emergency room and was severely dehydrated he passed away six days later but the staff was good

They were all great, from the caregivers, housekeepers, cooks to the RN's. I saw most of them as I was always visiting my Dad. They respected the residents & talked to them with respect.. They all showed great care not only for my Dad, but for the other residents... They called to let me know if something happened to Dad.

Do Not Put Your Loved One Here
My family and loved one didn't have the best experience with the community. During our loved one's stay, there was another resident who tried and succeed in getting into my loved one's room and slept in their room with our loved one. The call button wasn't even a button, it was a bell left on the end table. The staff couldn't hear the bell, so my loved one had to wait for a long time for assistance. Dirty conditions, fly in windows during winter, brace for broken back on end table across the room, controller for the bed filthy and broken, mom was trying to eat with a broken back by herself without any support, no heat at all and when we questioned it they offered a room with a covid patient in it that had heat. It's not recommended. My loved one had a broken back In a bed that was filthy and didn't work. No support. my loved one's brace even was taken off and thrown across the room where they couldn't get it. No heat in winter.

HomePlace has a way of making the resident feel special and good about themselves. They go on outings. They went to a Pumkin Patch and Tulip Tour to name a few. They help them make homemade soaps and ornaments, that they sell with all the proceeds going to the Alzheimer's Association. I can not say enough good things about HomePlace.

A place for dad
We went with HomePlace because I liked that it is split up into different areas so not all of the residents are in the same place. The facility is very clean and that was important for me. The staff are all extremely friendly and caring, they treat him respect and dignity. They are very flexible with visiting even with covid going on which we really appreciate. They seem to have a lot of great activities going on for the residents. For example they have birthday parties and yesterday they were having a valentines day party. Overall, I would definitely recommend them to others looking.

Homeplace is good care for those afflicted with memory loss. Centered around small, well designed neighborhoods means No isolation or long hallways to get lost in. Caring staff. Eyes and ears on residents who congregate within central living spaces. Good food. Glad mom is at Home Place.

My mom has been here just over six months. I looked at several facilities and decided on Homeplace. I'm happy I did. I feel she is well cared for and deep down she enjoys the attention. She is always clean, hydrated, cheerful and maybe even a little too well fed. This is what a memory care facility should look like. A big common area where residents stay and engage and staff have eyes on them, no long hallways to get lost in, no isolation; it's like a big old family home. While there are at times staffing struggles that are found in any long term care home, I feel that management keeps things stable, and the residents remain cared for. There are long term and even newer caregivers and support staff that are truly fantastic. Most have kind hearts and a genuine interest in caring for people, and are dedicated to the residents. This is really hard work emotionally and physically, and people stepping up to the plate to do it should be celebrated. There’s been positive enhancements in the activities staff and program, which is welcome. The food is declcious, and presented nicely. The executive director is responsive to questions or requests, and I like and respect her. Dementia is a terrible disease and you all have helped make this move a positive one in which I see my mom well supported and getting the care she needs, and even thriving just a little bit more each day. Thank you homeplace for caring for my mother.

Lovely experience!!
My husband had a lovely experience at this community. The staff where all very friendly and made sure that my loved ones needs where being met and that he was comfortable in his new living environment. Great Community Atmosphere
I love the community aspect of HomePlace! The layout of the building encourages residents to interact and congregate. It does not have the feeling of a hospital or an institution that many senior care facilities have. The staff is very friendly, caring, and they address problems immediately. The ratio of residents to caregivers seems to be about 12:1, but residents receive more attention as their quality of life decreases. My mother seems to like the food, but I wish I would see more of a variety of fresh fruits and veggies. Overall, a great place!
