Great Facilities
Was in town visiting and came by the carnival on Friday. The residents were having such a great time and the staff was great. It felt like home and comforting knowing our loved ones are in such wonderful hands. [name removed] was exceptionally amazing! Loved the face painting!

Horrible
I was a resident for four years. Unfortunately, I’ve had to deal with things of unimaginable discussed. This was/is a low income Medicaid facility so be prepared. The food is not good. The only good thing about the place is a few people who have work there and actually care.

Beware
When we were faced with one of the most difficult decisions regarding our mothers health & living situation. We were promised MANY things. And unfortunately NONE of them have been kept. When my mother was moved into the alf it was named Oqquire Meadows. They have since changed management FIVE times, as well as the management staffing. 2yrs later we STILL cant get her room cleaned weekly, they have either lost or ruined clothing, towels, and bedding whrn they have done her laundry. That has been taken over by family. Her heater has been broken for over 1 month and as of today 1/18/17 STILL not fixed. Bed buggs 3 times and I FINALLY ended up having to replace her mattress. She has gone upto 7 dys without showering or changing her clothes. And after 5pm and on weekends. There is no management or emergency process established. Unfortunately with the new choices waiver, options are limited and availability 1-2yrs waitting list. So IF you are asking WHY dont they just move her It's because we cant afford to pay the full out of pocket expense and she is on several waitting list of facilities who accept the New Choices Waiver. ( few do) So what else can we do, but do as much as we can to take care of her needs. And wait until we can move her from this poor example of a ALF. Everyday my mother lives in the facilitu. I dont feel good or that she is safe. Instead I live everyday full of regret & guilt

Great Staff!!
The Tradition Assisted Living unfortunately did not work very well for us. I did not feel a warmth feeling of the community and i could not feel that positive atmosphere. The way it also looked was more like a hospital building. The staff was very nice and the tour guide was very knowledgeable. I really liked the tour they provided they gave me a lot of information that i needed. Meal plans and activities are available.

Tips for evaluating your community options

Safety is a primary concern when seniors and their families look for Assisted Living. To ensure that a community meets your needs and expectations, we recommend that seniors and family members:

  • Tour the location: Touring allows you to speak to current residents and staff, and helps you confirm the accuracy of the community description and photos. Use our touring checklist to help guide your visit.

  • Confirm the availability of required services: Before you commit, make sure that the community offers all the services your senior needs, and that those services can be provided within your budget.

  • Know the Utah regulations for Assisted Living: Each state has its own laws and inspections processes to ensure seniors reside in a safe environment. Depending on your state, you may even be able to look up inspection reports for the community(ies) you’re considering. Learn more about Utah regulations for Assisted Living.

A Place for Mom has scored Tradition Assisted Living 1 out of 10 using our proprietary review score.

We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.

Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
