Washington and Nevada Consumer Health Data Privacy Policy

This section of the Privacy Policy applies solely to Washington and Nevada “Consumers,” as this term is defined pursuant to Washington’s My Health My Data Act and Nevada’s Consumer Health Data Privacy Law (collectively, the “Consumer Health Privacy Laws”).
Please note that certain terms used in this section of the Privacy Policy, such as “Affiliate,” “Consumer Health Data,” “Collect,” “Consent,” “Consumer,” “Personal Information,” “physical or mental health status,” “Process,” “Sale” and “Share” have the meanings given to such terms in the Consumer Health Privacy Laws, and not the common meanings of these terms in conversational English.
Below, please find a chart containing information about what categories of Consumer Health Data (“CHD”) we Collect, the purpose for such Collection, the categories of sources from whom we collect your CHD, the categories of CHD that we Share, and the categories of third parties and Affiliates with whom we Share the CHD.

As a Washington or Nevada Consumer, you have certain rights with respect to your CHD that we Collect or Process about you, including the rights to:
- Request confirmation of whether we are Collecting, Selling, or Sharing your CHD, and to access such data (including obtaining a list of all third parties and Affiliates with whom we have Shared or Sold your CHD, and obtaining an active email address from the parties receiving the information).
- Request deletion of any CHD that we hold about you.
- Request to review and request changes to any CHD that we hold about you.
- Withdraw your Consent to our Processing, Selling, or Sharing of your CHD.

- Specify the nature of your request (e.g., request to delete).
- Provide the first and last name, email address, and phone number that you think we have on file for you.
If we deny your request in whole or in part and you would like to appeal our decision, you may email us at WAHealthDataRights@aplaceformom.com or by calling to request that we reconsider your request. In your email to us, or on the phone, please be sure to:
- Specify the date and nature of your original request.
- Provide the first and last name, email address, and phone number that you think we have on file for you, the reason for your appeal, and any additional information you think might help us understand your request better.
We may change this section of the Privacy Policy from time to time. If we make changes, we will notify you by revising the effective date at the top, and, in some cases, we may provide you with additional notice (such as adding a statement to our homepage or sending you a notification). If we make material changes to the way in which we use or disclose information we collect, we will use reasonable efforts to notify you (such as by emailing you at the last email address you provided us, by posting notice of such changes on the Services, or by other means consistent with applicable law) and will take additional steps as required by applicable law.
